Referring Providers

Collaborative nutrition care for patients whose mental health, medications, and real-life circumstances are part of the picture.

At The Well-Minded Plate, I collaborate with healthcare providers to support nutrition in a way that improves mood, energy, cognitive capacity, and eating consistency. Many patients face nutrition-related challenges that impact their recovery and daily functioning, even when their care goals are unrelated to weight or body size. My approach supports realistic behavior change while integrating with the existing care team.

 

Why Refer?
 

Patients often continue to struggle with:

  • Cognitive fatigue, low energy, and difficulty with focus

  • Gut-related symptoms including IBS and digestive discomfort

  • Difficulty with motivation and follow-through around meals

  • Stress-driven, emotional, or disordered eating patterns

  • Appetite and metabolic changes related to psychiatric medications

How to Refer a Patient

1. Download and complete the referral form.

2. Fax to: (254) 221-9330 (HIPAA compliant e-fax)

3. I will contact the patient and take it from there.

Care coordination

With patient consent, I can share brief progress updates and coordinate on shared goals. This may include appetite changes, medication side effects, energy patterns, and eating consistency.


My care approach prioritizes clinical outcomes shaped by mental health stability, executive functioning, access to resources, and behavior goals that are realistic for everyday life.
